As a valued patient of Family Medicine Associates, we are pleased to inform you that we are joining Peak Primary Care, a Colorado Physician Partners practice, just a few minutes away at

I-25 and 104th Avenue. Our current practice location will close on July 24, 2024. 

 

We want to ensure you receive uninterrupted, exceptional care and have provided an update on what to expect as part of this transition.

 

UnitedHealthcare Medicare Advantage Members

As a UnitedHealthcare Medicare Advantage member, you will be re-assigned automatically to Dr. Ann Imhof at Peak Primary Care. Dr. Imhof is delighted to provide your care. Additionally, Peak Primary Care has several other experienced providers who can continue to support preventive care and chronic care needs. Please refer to the Peak Primary Care Practice Sheet enclosed with this letter to learn more about the services offered and to view the team of providers. We’re also happy to share that Amanda Black, FNP, will move to Peak Primary Care later this summer to continue to serve you.

 

Important Next Steps: 

  1. Please watch for a letter from your health plan and insurance card with Dr. Imhof as your provider. If you wish to see a different provider, please contact Peak Primary Care directly to make the change.

  2. Schedule your Annual Wellness Visit with Peak Primary Care at 303-452-2766. It is an important part of your ongoing care. Peak Primary Care may also contact you about meeting with a new provider and scheduling your Annual Wellness Visit.

  3. Please call Family Medicine Associates before July 24 if you have any questions or need help with a prescription refill.

We have partnered with Colorado Physician Partners and our patients can be seen nearby at Peak Primary Care, located a few minutes away at I-25 and 104th Avenue. The primary care providers at Peak Primary Care are available and excited to be your healthcare partner.

 

We’re also happy to share that Amanda Black, FNP, from Family Medicine Associates, will join the Peak Primary Care team later this summer. You can make appointments with her starting August 1. If you have questions or need to schedule appointments, please call Peak at 303-452-2766.

As a valued patient of Family Medicine Associates, we want to share an update that the practice will close on July 24, 2024. For the past 50 years, we have enjoyed caring for residents from across the Northern Front Range.

To help ensure there is no disruption to your care, we recommend connecting with a new primary care provider AS SOON AS POSSIBLE, as our schedule is almost full, we are trying to leave urgent spots open, but they fill up quick.  

Once you have choosen a new practice, please fill out the Medical Records release by clicking on the following link.  You must have a fax number to the new provider. This can be either found by calling the practice or looking on their website.
